Odontopaste endodontic dressing

July 07, 2017

Odontopaste is used to preserve the disinfection of a root canal. This disinfection may be achieved with sodium hypochlorite and EDTA rinses along with the standard endodontic filing.

Odontopaste is applied following the disinfection procedures. Odontopaste acts as an obturator of the root canal by occupying any space which may allow for bacteria to harbour or repopulate. In doing so, Odontopaste allows for healing and hence a reduction in inflammatory exudates so that a dry surface can be presented to the adhesives and sealers that are used to seal the canal. Odontopaste is intended for continuous use for less than 30 days.

Instructions for Odontopaste use

Odontopaste is a water-soluble, white, non-staining paste. Odontopaste contains clindamycin hydrochloride and triamcinolone acetonide. Clindamycin hydrochloride is a broad-spectrum bacteriostatic antibiotic while triamcinolone acetonide is a steroidal anti-inflammatory agent. The clindamycin hydrochloride acts as a preservative for the bulk of the obturating paste, preventing bacterial repopulation within the paste. The triamcinolone acetonide provides temporary pain relief.

Only when used in conjunction with proper endodontic procedures will Odontopaste assist with the relief of pain associate with acute pulpitis and periodontitis. Odontopaste is used as an interim pulpotomy dressing material for permanent teeth in cases of acute pulpitis where conventional pulpectomy and root canal treatment will follow.

Odontopaste cannot kill bacteria as the antibiotic is bacteriostatic and not bactericidal. Any longstanding infections will be unaffected by Odontopaste unless the systemic immune system has the capacity to infiltrate the biological cause. This is difficult in avascular root canals. For such infections, a calcium hydroxide based root canal paste is recommended for its active antibacterial action.

Odontopaste is designed to be used as a therapeutic dressing material in prepared canals after pulpectomy and root canal preparation to:

A. Temporarily reduce the pain of apical periodontitis
B. Reduce bacterial repopulation of root canals

Odontopaste can also be used in prepared root canals following trauma to reduce the incidence of inflammatory root resorption and in the reduction of existing inflammatory root resorption.

Odontopaste has negligible to no intrinsic staining of teeth, is not photosensitive and, as such, is not limited to subgingival application.

The device can be used on all age groups and body types. Odontopaste should only be applied by dentists.

Method of use

Prepare the tooth cavity according to the standard dental procedure for root canal medicament application. Ensure the Odontopaste has not exceeded its expiration date.

• Remove from the packaging, ensure the tamper evident caps are still intact before its first use, unscrew cap
• Dispense the required amount (sufficient quantity to fill the tooth cavity) onto a dental mixing pad
• Screw up cap, record when the tube was opened
• Apply the Odontopaste directly onto inflamed pulp with a cotton pellet and/or a lentulo spiral (dental tool)
• Ensure the root canal cavity is filled with the Odontopaste
• Seal the tooth as per standard dental procedure for applying a dental restoration
• Ensure the entire tube of Odontopaste is utilised prior to the expiration date
• More than one application may be required; the need for re-application should be decided by a dental professional

Contra-indication for Odontopaste use

• The Odontopaste is supplied non-sterile, the device is not designed to be sterile
• Each tube is designed to be used for numerous applications
• The Odontopaste should not be used if it has exceeded its expiration date
• Mixing surfaces and dental instruments should be cleaned in accordance with cross-infection protocols. Refer to relevant infection control protocols for dentistry
• Odontopaste should not be mixed with any product. Mixing with calcium hydroxide will impair the steroid and antibiotic

Warnings

• Each user should read the instructions for use and seek assistance with initial use if required
• Safety glasses, mask and gloves should be worn when handling the Odontopaste
• Odontopaste is not recommended for use in patients with allergy to clindamycin or lincomycin
• Odontopaste should not be used on patients taking concurrent doses of erythromycin as antagonism has been demonstrated between the two drugs in vitro
• Odontopaste should be used with caution on patients with a history of gastrointestinal disease, particularly those with colitis
• Odontopaste should be used with caution on nursing mothers as clindamycin has been shown to be present in breast milk
• Odontopaste should be used with caution on pregnant mothers as triamcinolone acetonide has demonstrated teratogenic effects in pregnant animals
